If your computer has a USB jack (most computers in the last few years do), you simply connect the UX16 to your USB connector on your computer and plug the MIDI in and out cables (shown shortened in the photo above - the actual length of the USB cable is 2 feet and the length of the MIDI cables is a little over 4 feet) into your keyboard. You will have to install a "driver" so that your computer knows about the UX16 and the driver is included with the interface on a CD-ROM. If there is anyone seriously interested in computer recording I would first check out the Edirol UA-4FX. It is a fully intergrated USB/Audio interface with 4 imputs including: 1 for MIDI, 1 RCA stereo imput, 1 for a dynamic mic/guitar and 1 for a condenser mic with the option to use phantom power, and whats even better is that is all USB bus powered. So there's no need for a power pack or bulky adapter, making it completely portable for you laptop people.

Note that if you're running a newer Mac (I have a MacBook Pro) you have to download the latest driver from Yamaha. And another gotcha is that you have to completely uninstall the driver they give you in the box before the new one will work. It's annoying as all get out, but the instructions for how to do the uninstall are on the CD so it's not -too- bad.
Anyway, once the right driver was installed this cable worked perfectly. I can record myself in GarageBand now while playing my DD-55 drum kit.

I've had no issues with this and love it. Simple to use and no issues using this interface with with my Yamaha CBX-T3.

The red light went off and the US 16 is not working anymore.

I encountered the same problem as Toni Haun, and needed to swap the MIDI IN port with MIDI OUT port for the adapter to function properly.
